Both. :-)

I think your confusion stems from the multiple different ways in which
it's possible to do ISDN. I recommend Dan Kegel's ISDN FAQ (a top search
candidate if you do a netsearch for ISDN) - it describes the difference
between ISDN terminal adaptors, sync ISDN cards, etc.
