Media problem.  Probably means a sector ID field could not be found
when writing.  If reformatting doesn't cure it, watch out for the next
bit-bucket.

Probably not.  I have no idea about Windows at all, but ISTR that the
docs mentioned it should be run on plainDOS.  There is rumoured to be
a Win version available now, too.  Again, hearsay.  I use dd(1) to
create my floppies. :)
