I have run into a similar problem; I don't have a real fix for it,
but do have an easier way to recover than re-booting:
 1) close and quit ppp
 2) su (become root)
 3) cu -l /dev/cuaa1 (my internal modem is on sio1)
 4) +++  (to get the modem's attention)
 5) (after modem says OK) ath (to hangup)
 6) exit (no longer root)
