I don't have any direct experience with this card. I *do* have experience
with other multiport 21x40 based cards.

Anyway, I'm not really sure what your question is. If this card works with
the same 'de' driver as the rest of the 21x40 cards, I assume the config
is the same as for other multiport cards. Here is the relevant part of the
configuration for a machine with one SMC Etherpower2 card (2 ports) and one
ZNYX ZX314 card (4 ports):
