This really sounds like a dodgy floppy.  Can you mount the floppy on
your 2.1.0 system ?  Can you fdisk /dev/rfd0.1440 ?

For example, my laptop happily writes floppies with absolute garbage.
I never get a write failure, but try to read it back ???? *boom*.
(It's not FreeBSD, it happens with DOS & Windoze too)
