Nice description of your hardware.  Do you have only 4 MB of memory?
Is your system overclocked?  Is the supported hardware setup with
unexpected IRQ's?

Moving onto the boot floppy.  Where did you get the boot.flp image?
CDROM? FTP? A friend?  How did you create the installation?  Did
you use the rawrite utility on MS-DOS system?  Did you use dd
on another UNIX or unix-like system?
