I had the same problem on a machine. It was the I/O card after all.
It was some cheap brand, and although it said it was 16650A etc, it still
couldn't do ppp. I replaced the card with a proper, designed 
communications card, and all worked well. Especiall cards with Winbond
chips on them are no good.!
