When I ordered my SCSI CD-ROM from Computability, they recommended
a company called Cables To Go at 1-800-225-8646.  You might want
to call them also just to check prices.  I have not had to use
them, but I might give them a call soon.  I was more interested in
a custom length internal cable though, so I don't know for sure 
that they do custom external cables.
