On a 1452, 1462, and now 1468, I see this problem frequently.  I have a short
handmade internal cable, active termination on the last drive in the chain,
and no external devices.  There are three things I've done at various times
to fix the problem: (1) switch to a 747 or 742; (2) shut off fast, synch, and
disconnect on all units; (3) switch to an NCR.

I have changed out the disks, upgraded the disk firmware, tried different
cables.  I admit that I have not tried a 1562 motherboard, but geez.

The smoking gun points squarely at the 946C (Rev E, latest firmware+BIOS).
