I've had exactly the same problem just yesterday. I made a post regarding it:

Subject: SCSI problems performing writes to FAST SCSI-2 drives

Sometimes the machine would produce some SCSI command error writing the
partition table. Others it would get past that stage and give the errors
you outline above.
  It really looks as though there is some problem with FreeBSD as Linux
Slackware v3.0.0 installs fine on the same hardware. I tried a 1542C and a 
AHA2940 on two machines, both with the same problems with FAST SCSI-2 drives,
but no problems at all under Linux.
