Hey there...

I am a bit new myself but perhaps I can help you out.  I have a very
similar (seems exact actually) video subsystem and was able to get
X11R6 working fine.

I too have a ATI WinTurbo with 2Mb Dram on board and the Mach64
accelerator.  My monitor is a Mag DXF15 which has exactly the same
sync rates as those you listed.

I am running FreeBSD version 2.1 from Walnut Creek CD-ROM (January
1996 version) and used the Mach64 accelerated driver under the novice
install XWindows menu.  I disabled the standard SVGA Server there
since I was not sure if it would conflict.  When I ran the config
program to configure XF86Config I just punched in what I had and
everything worked fine (I put in a custom line for the hsync values as
I was not sure if the 64.3KHz would make a difference (I think I have
used it under Linux 1.2.8 when I ran it but I decided not to chance it
here).

In any case, X11R6 is up and running beautifully and I have
resolutions available from 320 x 200 to 1280 x 1024.  They all are
accessible through <ctrl> - <alt> - <+> and <-> hotkeys.

If you like I can send you a copy of my XF86Config file so you can see
what is different from yours.  Let me know a little more specifically
what kind of problems your are having and maybe I can help out.
