Boy does this sound familiar ;)  I experienced this problem when I
replaced one of my two IDE drives (both are Conner drives).  I put the
new one in, and a week or so later noticed that FreeBSD no longer saw my
IDE controller.  It didn't occur to me at the time that it was the
replacement disk that was causing the problem, because both DOS and
Windows '95 worked just fine (they run off of the IDE drives).  It
wasn't until I started ripping components out of the box that I was able
to conclude that there was some problem with the two drives.  It turns
out that the newer Conner drive had to be jumpered a certain way to
coexist happily with the older drive.  Once I fixed that (after talking
with a Conner tech support person for 10 minutes) everything worked.  
If you've got two IDE drives, try disconnecting one of them and
rebooting the machine.  If you see that drive, you've probably got a
very similar problem to the one I had.
