Did you apply the "720k" fd0 patch that was posted several time ago? This
patch *mis*uses minor numbers for density information, instead of 
partitioning information, and is thus incompatible with nearly everything
around that uses the fd device. It is likely that the idea of the upper kernel
about the fd device does no longer correspond with the fd driver's opinion.

But you may of course have still problems with your sequence of patches.
