The NetBSD-1.0 arch/i386/pci/ncr.c is ancient and should be replaced
immediately. The newer versions are much, much more stable. I've
appended my back-port of the latest version of ncr.c (1.49--from
FreeBSD-current).

You'll also want to make sure you've got a moderately recent version
of the NCR chip. My chip has a 9428S date code and seems to work
ok. (I've had the odd problem, but nothing in the last couple of
months, and nothing since I upgraded to 1.49 a couple of days ago.)
I also had another controller with a 94xxS (sorry, I can't remember
the xx, but it was greater than 28) date code that worked all right.
On the other hand, a chip with the 9331N date code blew up consistently
when trying to write one particular file, and it did it under
several different version of the driver, both under FreeBSD and
NetBSD.

I don't claim to know anything about SCSI drivers, but Stefan has
been very helpful in the past, and I'm quite grateful to him for
the work he's done on this driver.
