There should be some hints in the FAQ (section 6 or so).

Go to /usr/src/i386/conf
cp GENERIC MYKERNEL
vi MYKERNEL
config MYKERNEL
cd ../../compile/MYKERNEL
make all
make install

The old kernel will be placed at /kernel.old , so if something goes
wrong, you can type "/kernel.old" (or "/kernel.GENERIC") at the boot
prompt.
