Some points of interest (I don't know for sure if they'll help):

The 1542B manual says that no more than 5 devices (including the 1542B)
should have terminator power.

It's very easy to "stretch" an internal scsi cable, making it verrrry dodgy.

There _must_ be 2 (two) bus terminators.  Either on the last internal and
last external device, or on the last device and on the 1542B.

According to Gravis (they produce UltraSound cards), some ISA machines don't
hack 16bit DMA very well, but I would assume that this means it doesn't
work at all.

Making sure that SCSI parity is enabled is probably a good idea.

Try adjusting the SCSI transfer rate on the 1542B.
