I am currently having the same problem (heh thought it would be simple,
but it's not :-( ). I also know of two other NetBSD users who have yet
to get the server running. I have it up and doing 640x480, 800x600, and
1024x768, but I have zero mouse control at this time.

I have ps/2 style connected mouse, but it probaly speaks like a microsoft
mouse since I can use the ms mouse driver with it. Also QAPlus tells me it
lives on IRQ 12, not 5 as the man page suggests. It autoconfigs fine with
the correct IRQ and address (according to my documentation here), but
later on down I see:

dev_mkdb: can't stat mouse

Now, I don't know if this message is normal or not since the man page claims
the probe routine does nothing at this time. I am attempting to use pms0
for the device and I have created /dev/pms0. I have tried mms0 and lms0
with failure; they don't autoconfig at all. If I do `cat /dev/pms0` it
says soemthign to the effect that the device is temporarily unavailable.

I've sent mail to the netbsd-help mailing list on this and I have not
received a response yet, but I'm going to comb the archives and look
at the sources. Failing that, perhaps I'll contact the author of the pms
driver.

If anyone out there has a ps/2 style connect mouse and you have it working,
please contact me, I'd be interested in exchanging mail...
